ABSTRACT

Introducción: la infección por COVID-19 afecta el tracto aerodigestivo superior a través de la enzima convertidora de angiotensina 2 (ECA2) y/o la proteasa transmembrana serina 2 (TMPRSS2). Sus manifestaciones agudas y secuelas han sido muy variadas y no todas están relacionadas con la intubación orotraqueal. El objetivo es describir las características sociodemográficas, clínicas y los hallazgos endoscópicos de los pacientes con síntomas laringofaríngeos posteriores a una infección por SARS-CoV-2 evaluados en el Hospital Militar Central y Hospital Universitario Clínica San Rafael entre marzo de 2020 y marzo de 2022. Materiales y métodos: estudio observacional de corte transversal con datos sociodemográficos, comorbilidades, necesidad de intubación orotraqueal, variedad de síntomas y sus hallazgos endoscópicos. Resultados: se recolectaron datos de 118 pacientes; la edad media fue de 51 años ± 14,4. El síntoma más frecuente fue la disfonía (69,5 %), seguido de la disnea (39,8 %). El 58,9 % requirió intubación orotraqueal y, de estos, la manifestación más frecuente fue disfonía por tensión muscular (DTM) y estenosis subglótica-traqueal. En el 41,1 % restante su hallazgo más frecuente fue la laringitis irritativa. Conclusiones: la COVID-19 tiene múltiples manifestaciones laringofaríngeas en relación con su mecanismo de infección e invasión en los tejidos de esta zona, de tipo inflamatorio y estructural, y no todos están relacionados con la intubación.


Introduction: COVID 19 infection affects the upper aerodigestive tract through angiotensin-converting enzyme 2 (ACE2) and/or Transmembrane serine protease 2 (TMPRSS2). Its acute manifestations and sequelae have been very varied, and not all of them are related to orotracheal intubation. The objective is to describe the sociodemographic and clinical characteristics and the endoscopic findings of patients with laryngopharyngeal symptoms after SARS-CoV-2 infection evaluated at the Hospital Militar Central and Hospital Universitario Clínica San Rafael between March 2020 and March 2022. Methods: Cross-sectional observational study, obtaining sociodemographic data, comorbidities, need for orotracheal intubation, variety of symptoms and their endoscopic findings. Results: 118 patients were collected; the mean age was 51 years ± 14.4. The most frequent symptom was dysphonia (69.5%), followed by dyspnea (39.8%). 58.9% required orotracheal intubation and of these the most frequent manifestation was muscular tension dysphonia (MTD) and subglottictracheal stenosis. In the remaining 41.1%, the most frequent finding was irritative laryngitis. Conclusions: COVID-19 has multiple laryngopharyngeal manifestations in relation to its mechanism of infection and invasion in the tissues of this area, as an inflammatory and structural type, and not all of them are related to intubation.

ABSTRACT

Resumen: Introducción: La pandemia por COVID-19 es una enfermedad de predominio respiratorio que ha afectado a nivel mundial y ha dejado más de 151 millones de casos, los cuales suelen requerir un manejo de ventilación mecánica con la intensión de controlar las vías aéreas superiores para la adecuada oxigenación de las personas con COVID-19, y que se reduzca así el riesgo de contagio para el personal médico. Objetivo: Describir el abordaje anestésico para la inducción y mantenimiento de una traqueoplastía derivado a estenosis traqueal secundaria por COVID-19. Material y métodos: Se aborda la descripción de un caso y los lineamientos que se han dado para el manejo de la estenosis traqueal. Conclusiones: El control oportuno anestésico y la instalación de férula laríngea son apropiados para el manejo de la estenosis traqueal secundaria al proceso de intubación mecánica prolongada por COVID-19.


Abstract: Introduction: The COVID-19 pandemic is a predominantly respiratory disease that has affected worldwide and has left more than 151 million cases, which usually require mechanical ventilation management with the intention of managing the upper airways for ventilation adequate oxygenation of people with COVID-19 and that reduces the risk of contagion for medical personnel. Objective: To describe the anesthetic approach for the induction and maintenance of a tracheoplasty derived from tracheal stenosis secondary to COVID-19. Material and methods: The description of a case and the guidelines that have been given for the management of tracheal stenosis are addressed. Conclusions: Timely anesthetic management and installation of a laryngeal splint is appropriate for the management of tracheal stenosis secondary to the process of prolonged mechanical intubation due to COVID-19.

ABSTRACT

Abstract Stenting for lower tracheal stenosis is a tricky situation and for the safe conduct of anesthesia, it is imperative to maintain spontaneous respiration. Airway topicalization is routinely recommended for anticipated difficult airway. We report a case of upper airway obstruction following lidocaine nebulization in a patient to be taken for tracheal stenting for lower tracheal stenosis. We would like to highlight that close monitoring of the patient is advisable during airway topicalization to detect any airway obstruction at the earliest and how fiberoptic intubation can play a pivotal role to secure the airway in an emergency scenario.

OBJECTIVES@#To study the role of bronchoscopy in slide tracheoplasty.@*METHODS@#A retrospective analysis was conducted on the diagnosis and treatment of four children with tracheal stenosis admitted to Hunan Provincial People's Hospital from 2017 to 2020. The role of bronchoscopy was summarized in the preoperative evaluation, intraoperative positioning and measurement, and postoperative wound evaluation and treatment during slide tracheoplasty.@*RESULTS@#Bronchoscopy evaluation before slide tracheoplasty showed that 3 of the 4 children had complete trachea rings, 2 had pulmonary artery sling, and 2 had multiple stenosis. Slide tracheoplasty was performed in the hospital on 3 children, and the midpoint of the stenosis segment was judged under bronchoscopy, and the length of the stenosis segment was measured, which assisted in the resection of the stenosis segment of the trachea. The pathogens were identified by lavage after the surgery. One child who developed scar traction 9 months after slide tracheoplasty in another hospital was improved by interventional treatment under bronchoscopy. Mucosal changes were found under bronchoscopy in 2 children 4 days after surgery, and the treatment plan was adjusted. One month after surgery, 2 children had granulation hyperplasia, which was improved by cryotherapy under bronchoscopy. One child abandoned treatment due to anastomotic necrosis and died. Three survivors were followed up for over 6 months with good prognosis, but all had tracheobronchial malacia.@*CONCLUSIONS@#Bronchoscopy can be used for the management of slide tracheoplasty in children with tracheal stenosis, which is helpful to postoperative rehabilitation and follow-up.

RESUMEN Introducción: La enfermedad por SARS-CoV-2 se convirtió en un importante problema de salud pública. El 5 % de los pacientes requirió intubación endotraqueal por insuficien cia respiratoria aguda hipoxémica, y se generó un aumento de consulta por estenosis traqueal. Se realizó el trabajo con el objetivo de expresar los resultados de la cirugía de resección traqueal en pacientes con estenosis post COVID 19. Material y métodos: Estudio prospectivo, observacional, analítico. Se incluyeron 11 pacientes. La evaluación prequirúrgica y postquirúrgica fue la misma en todos los pacientes. Se valoró en el postquirúrgico la ventilación y la fonación hasta los 30 días. Se utilizó la clasificación de Clavien y Dindo para calificar a las complicaciones post-quirúrgicas, con seguimiento hasta los 60 días postquirúrgicos. Análisis estadístico: se aplicó prueba de Wilcoxon para comparar los resultados. Resultados: El 27.2 % de los pacientes tuvieron complicaciones postquirúrgicas. Fue estadísticamente significativa la comparación de la ventilación entre el prequirúrgico y el postquirúrgico (p<0.05) con mejoría en el postquirúrgico. Al comparar la fibrobron coscopía prequirúrgica con la postquirúrgica (diámetro de la luz traqueal) también el resultado fue estadísticamente significativo (p <0.05). Conclusión: Los resultados obtenidos son similares a los expresados en la literatura. La cirugía de resección traqueal es un procedimiento seguro y efectivo y debe ser considerada como tratamiento de primera línea para la estenosis traqueal.


ABSTRACT Introduction: The disease caused by SARS-CoV-2 has become a significant public health issue. 5 % of the patients required endotracheal intubation due to acute hypox emic respiratory failure, leading to an increased number of consultations for tracheal stenosis. This work was done with the aim of presenting the results of tracheal resection in patients with post-COVID-19 stenosis. Material and methods: Analytical, prospective, observational study. 11 patients were included. The preoperative and postoperative evaluation was the same for all patients. Post-surgical ventilation and phonation were assessed up to 30 days. The Clavien and Dindo classification was used to grade post-surgical complications, with follow-up extended up to 60 post-surgical days. Statistical analysis: the Wilcoxon test was used to compare the results. Results: 27.2 % of the patients had postoperative complications. The comparison of pre- and postoperative ventilation (p < 0.05) was statistically significant, with improve ment in the postoperative period. When comparing pre- and postoperative fiberoptic bronchoscopy (tracheal lumen diameter), the result was also statistically significant (p < 0.05). Conclusion: The results obtained are similar to those expressed in the literature. Tra cheal resection is a safe and effective procedure and should be considered as first-line treatment for tracheal stenosis.

ABSTRACT

Introducción: la infección por COVID-19 ha dejado más 400 millones de personas infectadas hasta la fecha, y entre un 10 %-15 % requiere intubación orotraqueal. Esto ha desencadenado una ola de secuelas en la vía aérea a largo plazo, que han aumentado la morbimortalidad posinfecciosa. El objetivo de este trabajo fue describir las características demográficas, clínicas y los posibles factores de riesgo de los pacientes con estenosis laringotraqueal (ELT) posintubación por COVID-19 en el Hospital Universitario Clínica San Rafael y el Hospital Militar Central en Bogotá, Colombia. Materiales y métodos: estudio observacional descriptivo de serie de casos retrospectiva que recolectó datos sociodemográficos, comorbilidades, tiempos de intubación, características de estenosis, resultados de estudios endoscópicos, manejos ofrecidos y resultados. Resultados: se registraron 25 pacientes, 15 hombres (60 %) y 10 mujeres (40 %), con tiempos de intubación promedio de 15,7 días. Los subsitios más comprometidos fueron la tráquea (68,2 %) y la subglotis-tráquea (22,7%). La mayoría de pacientes requirió más de una intervención. La dilatación con balón y traqueostomía fueron los procedimientos más realizados. Conclusiones: la ELT es una complicación de la intubación orotraqueal que, asociada con la vasculitis, inflamación y necrosis coagulativa de la vía aérea, ha aumentado en los casos de infección grave por COVID-19


Introduction: COVID-19 infection has left more than 400 million people infected to date, and between 10%-15% require orotracheal intubation. This has triggered a wave of long-term airway sequelae, which have increased post-infectious morbidity and mortality. Objectives: To describe the demographic and clinical characteristics and possible risk factors of patients with laryngotracheal stenosis (LTE) post-intuba-tion by COVID19 at the Hospital Universitario Clínica San Rafael and the Hospital Militar Central in Bogotá, Colombia. Methods: Descriptive observational study of retrospective case series, collecting socio-demographic data, comorbidities, intu-bation times, stenosis characteristics, results of endoscopic studies, management offered and results. Results: 25 patients were collected, 15 men (60%) and 10 wo-men (40%), with average intubation times of 15.7 days. The most compromised subsites were the trachea (68.2%) and subglottis-trachea (22.7%). Most patients re-quired more than one intervention. Balloon dilation and tracheostomy were the most performed procedures. Conclusions: LTE is a complication of orotracheal intuba-tion, associated with vasculitis, inflammation and coagulative necrosis of the airway, has increased in cases in severe COVID-19 infection.

ABSTRACT

El complejo ring-sling es una asociación entre el sling de la arteria pulmonar y la estenosis traqueal congénita por anillos traqueales completos. El sling de la arteria pulmonar es una forma rara de anillo vascular dentro de las cardiopatías congénitas. Se presenta el caso clínico de un niño con estridor laríngeo asociado a dificultad respiratoria evaluado en otro centro, donde se realizó endoscopia respiratoria y se observó compresión traqueal extrínseca. Ante la sospecha clínica de anillo vascular, se solicitó angiotomografía computada (angioTC) y se confirmó diagnóstico de sling de arteria pulmonar. Fue derivado a nuestro hospital y durante la intervención quirúrgica se realizó nueva endoscopia respiratoria y traqueobroncografía. Se llegó al diagnóstico de estenosis traqueal congénita con bronquio derecho accesorio (pig bronchus) y anillos vasculares completos, ambos reparados en el mismo acto quirúrgico.


The ring-sling complex is an association between pulmonary artery sling and congenital tracheal stenosis. Pulmonary artery sling is a rare form of vascular ring in congenital heart disease. The clinical case of a child with inspiratory laryngeal stridor associated with respiratory distress is presented, evaluated in another center where respiratory endoscopy was performed, observing extrinsic tracheal compression. Given the clinical suspicion of a vascular ring, CT angiography was requested, confirming the diagnosis of pulmonary artery sling. He was referred to our hospital and during the surgical intervention a new respiratory endoscopy and tracheobronchography were performed, reaching the diagnosis of congenital tracheal stenosis, right accessory bronchus (pig bronchus) and complete vascular rings, both repaired in the same surgical act.

ABSTRACT

Objective:To explore the clinical typing, diagnostic method and treatment plan of congenital tracheal stenosis (CTS) combined with communicating bronchopulmonary foregut malformation (CBPFM) in children.Methods:The clinical data of 2 children with CTS and CBPFM who were treated in the Center for Respiratory Intervention of Children′s Hospital Affiliated to Shandong University in May 2021 and January 2022 were retrospectively analyzed.Studies were retrieved from domestic and foreign databases, so as to summarize the clinical characteristics of CTS complicated with CBPFM and investigate the typing method of CBPFM.Results:One patient was a 4-year-old girl, who sought the medical advice due to " recurrent cough and asthma for more than 4 years" . She was diagnosed with typeⅡ CBPFM at the right side and CTS.Surgical thoracoscopic right pneumonectomy plus oesophageal repair was performed.The other patient was a 7-month-and-2-day-old female, who visited the hospital for " difficult eating, dyspnea and purple lip cyanosis for 7 months" . This patient was diagnosed with typeⅡ CBPFM at the left side and CTS.Slide tracheoplasty and left pneumonectomy+ oesophageal repair were performed successively.Eight English and one Chinese studies were collected.Twenty-one children with CBPFM and 12 children with CTS and CBPFM were included.Eleven CTS cases with sufficient diagnostic evidence were complicated with typeⅠA and typeⅡCBPFM.Conclusions:CTS and CBPFM can lead to severe wheezing and dyspnea.Clinicians should enhance their awareness and be more cautious.There may be a potential link between CTS and typeⅠA and typeⅡCBPFM, and further investigation is required.

ABSTRACT

Objective In view of the situation that tracheal atrophy causes the overall airway size to become smaller in the elderly, effects of the airway wall surface on reconstruction of a narrow airway and the airflow field under different respiratory conditions were investigated. Methods A three-dimensional (3D) model of human airway was established by using Mimics, and flow field in the airway was simulated by computational fluid dynamics (CFD) method. The inner wall pressure and the distribution of airflow were analyzed and compared under different breathing states. Results Under different respiratory states, the pressure of endotracheal wall was relatively uniform in the endotracheal wall, but decreased significantly in air inlet of the bronchial stenosis segment, and reached negative pressure near the narrowest area. The airflow velocity decreased from the center of the pipe to the boundary layer, and the velocity reached the maximum at the narrow area. Vortex was generated when airflow passed through the narrow area, and the larger the inlet flow velocity was, the larger the positive pressure and negative pressure were, the more obvious the pressure drop at the narrow area was, and the more obvious the vortex phenomenon was. Conclusions The constriction of the airway stenosis area caused by negative pressure will lead to the patient’s dyspnea, and the eddy current will cause the airway wall to be affected by the aerodynamic shear stress and may damage the airway wall mucosa. Therefore, understanding of the pressure distribution and velocity distribution in the narrow airway can provide references for clinical diagnosis and treatment of such diseased airways.

ABSTRACT

We describe the diagnosis and treatment of a very premature female infant with gastroschisis complicated by tracheostenosis. The pregnant woman, whose fetus was diagnosed with gastroschisis by ultrasound at 22 weeks in a local hospital, was admitted to the Second Affiliated Hospital of Wenzhou Medical University at 28 +1 weeks with oligohydramnios. Ultrasound after admission confirmed the previous diagnosis. A live baby girl was born by vaginal breech delivery at 29 +1 weeks after spontaneous rupture of the membranes. Because of the unstable oxygen saturation, the neonate finally received Silo in the delivery room prior to the closure of abdominal fissure 7 d after birth, and during the placement difficult endotracheal intubation was evident. She was diagnosed with having congenital tracheal stenosis via chest CT scans with 3-dimensional reconstruction 3 weeks after birth and received transbronchoscopic balloon dilatation at 3 months after birth. During the 2-year follow-up, she grew well without any complications.

Objective:To study the changes of stenotic segment trachea diameter, trachea length and carina angle before and after slide tracheoplasty.Methods:From January 2010 to January 2018, 77 children with congenital tracheal stenosis(female=36) with complete clinical data accepted slide tracheoplasty in Shanghai Children's Medical Center were selected as the observation group. The diameter of stenosis segment, the length of stenosis segment, the full length of trachea and carina angle were measured by CT image, and the ratio of stenosis segment length to full trachea length was calculated. A group of children with normal trachea morphology and the same sample number, and they had no significant difference in age, height and weight were selected as the control group.Results:The median operative age of 77 children in the observation group was 1.3(1.1, 1.9)years old, the height was 76.0(72.0, 83.0)cm, and the weight was 9.0(7.9, 10.8)kg. The age, height and weight of 77 children in the control group were 1.5(1.0, 2.0) years old and 78.0(71.0, 85.0) cm, 9.2(7.9, 11.0) kg respectively. After slide tracheoplasty, the diameter of trachea in the stenotic segment of children increased from 2.7 mm to 4.4 mm, which was increased by 63.0%( P<0.001), but still smaller than that of children in the control group(6.1 mm). The average length of trachea was shortened by 0.7 cm( P<0.001), and there was no significant difference between the total length of trachea(5.4 cm) and the total length of trachea(5.5 cm). The postoperative carina angle decreased by 27.9°( P<0.001), there was no significant difference in postoperative carina angle between the observation group(93.0°) and the control group(90.7°). Conclusion:The diameter of reconstructed trachea in children with CTS is significantly increased, the length and carina angle of trachea become smaller after slide tracheoplasty. The morphology of trachea is more similar to children in the control group.

Introducción. La causa más común de estenosis laringotraqueal benigna es iatrogénica, secundaria a intubación orotraqueal. El manejo quirúrgico sigue siendo la alternativa que ha mostrado mejores resultados a largo plazo. El objetivo de este estudio fue analizar la experiencia en el manejo quirúrgico de la estenosis laringotraqueal durante 15 años en un hospital de alta complejidad en Colombia.Métodos. En este estudio se revisaron las historias clínicas de todos los pacientes tratados con reconstrucción de estenosis traqueal benigna, entre los años 2005 y 2020. Para el análisis estadístico se usaron métodos de estadística descriptiva, con análisis de frecuencias y medidas de tendencia central o de dispersión.Resultados. Se identificaron 38 pacientes con estenosis laringotraqueal, con un grado variable de estenosis. La nasofibrolaringoscopia fue bien tolerada y segura para determinar el grado y la longitud de la estenosis. El uso de tomografía sirvió para determinar la extensión y las características anatómicas. Los resultados del presente estudio son similares a los de la literatura en cuanto a complicaciones, mortalidad y falla de la anastomosis. Las complicaciones más frecuentemente reportadas son reestenosis, infección del sitio operatorio, lesión nerviosa y fístula, que en general se presentan en un tercio de los pacientes. Conclusiones. La experiencia de manejo quirúrgico de estenosis laringotraqueal en este hospital permite concluir que la reconstrucción traqueal es una opción segura en nuestro medio. Las tasas de éxito y de falla del tratamiento son equiparables a las reportadas en la literatura.


Introduction. The most common cause of benign laryngotracheal stenosis is iatrogenic, secondary to orotracheal intubation. Surgical management continues to be the alternative that has shown better long-term results. The objective of this study was to analyze the experience in the surgical management of laryngotracheal stenosis for 15 years in a high-level complexity hospital in Colombia.Methods. Medical records of patients treated with reconstruction of benign tracheal stenosis between 2005 and 2020 were reviewed. Descriptive statistical methods were used, with frequency analysis and measures of central tendency or dispersion.Results. Thirthy-eight patients with laryngotracheal stenosis were identified, with a variable degree of stenosis. Nasofibrolaryngoscopy was well tolerated and safe to determine the degree and length of the stenosis. The use of tomography served to determine the extension and anatomical characteristics. The results of the present study are similar to those of the literature in terms of complications, mortality and failure of the anastomosis. The most frequently reported complications are restenosis, surgical site infection, nerve injury, and fistula, which generally occur in one third of patients.Conclusions. The experience of surgical management of laryngotracheal stenosis in our hospital allows us to conclude that tracheal reconstruction is a safe option in our environment. Treatment success and failure rates are comparable to those reported in the literature.

ABSTRACT

Objective:To investigate the common types, surgical treatment and effects of tracheal stenosis in children.Methods:A total of 23 children with tracheal stenosis in our hospital from December 2017 to August 2020 were retrospectively reviewed, including 14 males and 9 females. The mean age at operation was(8.9±5.8)months(range: 2-3 months) and the mean weight was(6.4±2.3)kg(range: 4.2-10.5 kg). The common types of tracheal stenosis were complete tracheal ring in 9 children, tracheomalacia in 10 and subglottic membranous annular hyperplasia in 4. The type of congenital heart diseases included 10 patients of pulmonary artery sling, 1 of tetralogy of Fallot, 5 of ventricular septal defect, 1 of pulmonary atresia, and 1 of right aortic arch with aberrant left subclavian artery. Slide tracheoplasty was performed in 9 patients, external splint in 8, endotracheal stent in 2 and tracheal dilation in 4. All children were followed up after 1, 3, 6, and 12 months of operation with CT and bronchoscopy.Results:There was 1 death in all 23 patients and the mortality was 4.3%, which died of granulation tissue hyperplasia after slide tracheoplasty. Reoperation was performed in 1 patient with endotracheal stent. All patients were followed for 1 to 24 months. Clinical symptoms of tracheal stenosis disappeared and the results of CT and bronchoscopy were satisfied.Conclusion:Slide tracheoplasty is the effective surgical method for complete trachea ring. 3D printing bioresorbable external splint is a promising method for the treatment of tracheomalacia.

ABSTRACT

Objective:To investigate the efficacy of descending aortic translocation in relieving the compression of trachea or bronchus caused by aortic deformity.Methods:From January 2017 to July 2019, a total of 11 patients with distal trachea or proximal bronchial stenosis caused by aortic deformity were treated with descending aortic translocation. Cardiac CT and fiberoptic bronchoscopy were performed before surgery. The median age was 55(23-540) days, and the body weight was 4(2.1-9.0)kg. Five patients had a special type of vascular ring(left aortic arch with right descending aorta, small aortic window with funnel chest; left aortic arch with right descending aorta and right artery ligament, vagus right subclavian artery, combined with trachea, carina and left and right bronchial stenosis in 1 case; Right aortic arch with left descending aorta, combined with tracheomalacia stenosis in 1 case; Right aortic arch with left descending aorta combined with broad tracheal stenosis and left pulmonary dysplasia in 1 case) compressed tracheal or bronchial tube in 5 cases. Three patients with left main bronchus constriction after traditional arch disconnection surgery. 3 patients with left main bronchus stenosis before coarctation or interrupted aortic arch.Surgical methods: Descending aortic translocation was performed through a midline sternotomy with cardiopulmonary bypass and deep hypothermia.The proximal descending aorta was transected distal to the left subclavian artery, proximal sutures were performed, and the distal brought up though the transverse sinus caudad to the right pulmonary artery and tracheal carina, and anastomosed in end-to-end fashion to the ascending aorta, and simultaneous correction was performed with intracardiac malformations, such as airway plasty was performed at the same time without improvement after compression of tracheal stenosis.Results:There was no death in the whole group. Median cardiopulmonary bypass was 180(136-337)min with an median aortic cross-clamp time of 51(30-84)min; Median absence of perfusion to the descending thoracic aorta 34(21-50)min .Tracheal compression was effectively relieved in 5 patients, and airway plasty was performed simultaneously in 6 patients due to persistent tracheal softening and stenosis. All patients had resolution of symptoms.There was only one case anastomotic stenosis after descending aortic translocation.The mean follow-up was(18.0±9.4)months.Conclusion:Descending aortic translocation can effectively relieve this kind of pressure due to aortic deformity.

ABSTRACT

Objective:To evaluate the efficacy of constant flow oxygen supply via laryngeal mask airway combined with a thin tube at the distal end of airway stenosis for intraoperative ventilation in the patients with severe tracheal stenosis undergoing bronchoscopy. Methods:Forty patients of either sex, aged 18-70 yr, scheduled for comprehensive interventional therapy with tracheoscope for severe tracheal stenosis, were allocated into 2 groups ( n=20 each) by a random number table method: test group and control group.After routine anesthesia induction, intermittent positive pressure ventilation was supplied via a laryngeal mask airway linked to four-way connector and anesthesia machine.In test group, a 6Fr suction tube was inserted through the four-way connector and placed at the distal end of the stenosis, with 1 L/min flow oxygen supply.Before ventilation with the thin tube (T 0), at 30 min after start of mechanical ventilation (T 1), and at the end of operation (T 2), SpO 2, P ETCO 2, mean airway pressure (Pmean), and minute ventilation (MV) were recorded, monitoring of lung ventilation was implemented using electrical impedance tomography, and the area percentages of the centre of ventilation (CoV), dependent silent spaces (DSS), and non-dependent silent spaces (NSS) were recorded.Blood gas analysis was performed at T 1 to record pH value, PaO 2 and PaCO 2.Hypoxemia was recorded during ventilation. Results:Compared with control group, MV, Pmean and area percentage of NSS were significantly increased and area percentage of DSS was decreased at T 1, 2, P ETCO 2 was decreased and SpO 2 and area percentage of CoV were increased at T 1, pH value and PaO 2 were increased, and PaCO 2 and the incidence of hypoxemia were decreased in test group ( P<0.05). Conclusion:Combination of laryngeal mask airway and a thin tube at the distal end of airway stenosis for constant flow oxygen supply can improve the ventilation efficacy by increasing the oxygen concentration at the distal end of the stenosis and by reopening the collapsed lung tissue at the distal end during interventional therapy with tracheoscope in the patients with severe tracheal stenosis.

RESUMEN Fundamento: el incremento del número de casos con estenosis laringotraqueal constituye una preocupación en el presente y futuro de la práctica médica. Objetivo: caracterizar la estenosis laringotraqueal. Métodos: se realizó un estudio de tipo analítico longitudinal retrospectivo, con el objetivo decaracterizar la estenosis laringotraqueal en el Hospital Militar Clínico Quirúrgico Docente Dr. Octavio de la Concepción y de la Pedraja de la provincia Camagüey, desde enero de 2014 a enero de 2019.El universo estuvo compuesto por 12 pacientes con diagnóstico de estenosis laringotraqueal. Resultados: en el estudio el promedio de edad fue de 40,6 años, en cuanto al sexo hubo igualdad para ambos, la principal causa de estenosis laringotraqueal fue postintubación, la localización de la estenosis más frecuente fue la subglótica. La severidad de la estenosis fue proporcional a los días de permanencia de la ventilación artificial. La mayoría de los pacientes presentaron un grado III de la estenosis según la clasificación de Cotton. Conclusiones: las estenosis laringotraqueales constituyen una complicación cuyas causas más frecuentes son evitables. Los traumatismos externos y con mayor frecuencia son los traumas endógenos por intubación y en su mayoría pueden ser eludibles. La epidemiología ejerce poca influencia en la aparición de la estenosis, el tiempo de permanencia del tubo, fue un factor asociado a el mayor número de enfermos.


ABSTRACT Background: the increase in the number of cases with laryngeal-tracheal stenosis is a concern in the present and future of medical practice. Objective: to characterize laryngeal-tracheal stenosis. Methods: a retrospective longitudinal analytical study was carried out, with the objective of characterizing the laryngeal-tracheal stenosis at the Octavio de la Concepción y de la Pedraja Teaching and Surgical Clinical Military Hospital of Camagüey from January 2014 to January 2019.The universe was composed by 12 patients diagnosed with laryngeal-tracheal stenosis. Results: in the study the average age was 40.6 years, in terms of gender there was equality for both, the main cause of laryngeal-tracheal stenosis was post-intubation, the location of the most frequent stenosis was subglottic. The severity of the stenosis was proportional to the days of permanence of artificial ventilation. The majority of patients presented grade III stenosis according to Cotton's classification. Conclusions: laryngeal-tracheal stenosis is a complication whose most frequent causes are avoidable. External trauma and most often endogenous trauma by intubation are the most frequent and can be eludedmostly. The epidemiology exerts little influence on the appearance of stenosis, the length of time of the tube, was a factor associated with the greater number of patients.

La estenosis traqueal adquirida es muy poco frecuente en la edad pediátrica y puede responder a múltiples causas; la estenosis posintubación y la secundaria a traqueotomía son las más comunes.El manejo de la estenosis traqueal adquirida sigue siendo controvertido. El tratamiento endoscópico proporciona el alivio inmediato de los síntomas, pero se asocia a una alta tasa de recurrencia, y el uso de prótesis endoluminales puede incrementar la longitud de la estenosis y complicar una cirugía futura. Por el contrario, se han documentado buenos resultados a largo plazo con la resección traqueal con anastomosis primaria.Se describe nuestra experiencia con la resección traqueal con anastomosis término-terminal para el tratamiento de la estenosis traqueal posintubación en 8 pacientes.


Acquired tracheal stenosis is a very infrequent lesion in the pediatric age group and may be due to diverse causes, being post-intubation stenosis and stenosis secondary to tracheostomy the most common ones.The management of acquired tracheal stenosis remains controversial. Endoscopic treatment provides immediate relief of symptoms but it is associated with a high recurrence rate, and the use of endoluminal prostheses can increase the length of the stricture complicating future surgery. Conversely, good long-term results have been documented with tracheal resection and primary anastomosis.We describe our experience with tracheal resection followed by end-to-end anastomosis for the treatment of post-intubation tracheal stenosis in 8 patients.

Abstract Purpose: To develop a simpler animal model for benign tracheal stenosis and introduce a low-cost household endoscope for postmodeling endotracheal evaluation. Methods: Twenty rabbits were randomly divided into a model group (15 rabbits, subjected to transoral nylon brush scraping of the trachea) and a mock group (5 rabbits, merely exempted from scraping), a household endoscope was then introduced for weekly endoscopic examination. Meanwhile, other 15 rabbits (modeling like the model group) underwent batch tracheal resection at different postintervention times for pathological analysis. Results: The model group presented a low mortality and few complications. The endoscope could obtain adequate images for stenosis assessment, which showed that the models presented homogeneous injury after scraping and developed a mature scar stricture at 28 days postoperatively with a mean stenosis degree of 65.9%, and 71.4% (10/14) above Myer-Cotton's grade II. The pathological findings were consistent with the clinicopathological process of human. No stenosis was found in mock group. Conclusion: The modified model is simpler, minimally invasive and reliable, while the household endoscope is competent for model's follow-up, providing easily accessible and useful tools for facilitating more extensive studies of benign tracheal stenosis.

Objective@#To assess the outcomes of partial cricotracheal resection (CTR) and extended cricotracheal resection (ECTR) for severe laryngotracheal stenosis.@*Methods@#From November 2009 to September 2017, 18 patients underwent CTR and ECTR at the Department of Otorhinolaryngology Head and Neck Surgery, Tangdu Hospital, Air Force Medical University for severe laryngotracheal stenosis were reviewed retrospectively. There were 12-male and 6-female patients, with the age ranged from 4 to 56 years (median 25 years). The causes were postintubation in 11 cases, cervical trauma in 4, idiopathic in 3. The stenosis located in subglottic and tracheal (n=12), glottic and subglottic and tracheal (n=3), subglottic (n=2), and glottic and subglottic (n=1). Two patients had concurrent unilateral vocal cord palsy.One patient had undergone previous endoscopic balloon dilation and 8 patients had previous laryngotracheal reconstruction. The stenosis was graded according to modified Myer-Cotton classification as follows: Ⅲb (n=1), Ⅲc(n=1), Ⅳa (n=2), Ⅳb (n=12), Ⅳc (n=2). The surgical outcomes and complications were recorded.@*Results@#Among 18 patients,11 of the 12 patients undergoing CTR were decannulated. Five of the 6 patients undergoing ECTR were decannulated. Resected airway length ranged from 1.5 to 4.0 cm (median 2.8 cm). Surgical complications included infection of incision wound in 2 cases, anastomotic granulation in 2, cervical subcutaneous emphysema in 1, aspiration in 1, and unilateral arytenoid prolapse in 1. No recurrent laryngeal nerve injury or tracheoesophageal fistula occurred. The median follow up was 11 months.@*Conclusions@#CTR is efficient for severe subglottic and upper tracheal stenosis while ECTR is efficient for subglottic stenosis extended to the glottis. Both procedures also provide a salvage therapy for patients with previous failed treatments.
